Prayer wheels are used by Buddhists who turn them clockwise to accumulate merit and help all sentient beings purify their karma. They can commonly be seen in Buddhist monasteries and monuments, particularly in the Himalayan region.
This is a miniature replica of such a prayer wheel with the heart mantra carved on to it which can now be worn as a pendant. Spinning the wheel clockwise amplifies the blessings of the sacred mantra carved on it.
